While practicing manual docking procedures, astronauts on the Russian Space Station Mir lost control of a Progress resupply ship. The ship then crashed into the space station, causing the cabins to depressurize and damage to the station's solar panels.

As far as I know, that's happened only once--a couple of russian satellites in nearly the same orbit bumbed into each other a few years back. During the 1980s, I believe, but I can't recall the exact date. Sorry.

Aside from a couple of rendezvous and docking accidents, probably not. There was one Russian geostationary satellite that got knocked off course for unknown reasons, but no other satellite was ever identified as having hit it.
