It was based on a couple of stories in a book called Rope Burns by Jerry Boyd and published under the name F.X. Toole. He died in 2002 following heart surgery.

It is my understanding that the stories were based upon actual events but they were fictionalized. The stories in Rope Burns were "Million $$$ Baby" "The Monkey Look" and "Frozen Water" I haven't actually read any of the book and I'm not sure if the Bear's punch and the subsequent injury were written by Toole or the screenwriter Paul Haggis.
