Surgery. A plastic surgeon would have to (cross your legs, squeamish readers) amputate your penis in the middle and remove the amount of flesh you want excised, then reconnect the flesh, nerves and urethra. The latter part of this procedure is what was done fairly successfully to John Wayne Bobbitt after his wife Lorena viciously mutilated him.
