1) He has blue eyes.
2) He met Lupin when he was thirteen and just starting his Third Year. Specifically, he met Lupin on the train to Hogwarts. However, Harry probably met him long before that when he was a baby since Lupin was one of James' best friends.

4) Wormtail, a nickname given to him as a schoolboy and referring to the fact that his Animagus form of the rat has a wormlike tail. I'm sure Black also has a few nicknames of his own for Pettigrew!
5) We know they hid out at a house in the village of Godric's Hollow when Voldemort was hunting them down but it's never been revealed where they actually lived before then. I doubt that was their own house since they wouldn't have hid in such an obvious place.
