for the court; i think you are talking about a CIS that is to be attached to a child support order or modification request.

those forms are
1) available at the county court clerks office (free just go pick them up). they can show you what to do with the forms and how to fill them out if you need help.
2) free to print out on line (just go to your states site, then click on judicial, then find the forms you need).

if you still need help, you can call your local Legal Aid office to find out if they can help. Legal aid is in every state and most counties. they go by sliding scale so its free to most people. call your county court clerks office for the number.

If you don't have an attorney, then use your tax returns, and follow the directions on the form. You may need to estimate certain items, so just be realistic. Try tracking your expenses for the past six months for child care, food, clothes, school, movies, etc. and just give a rough estimate. Remember that the court expect the child to be taken care of in the same manner before divorce as after, so those are the numbers you should be using. Good luck!
