2) Your weak qualities and are you improving/working on your weaknesses
3) How do you work in a team?
4) Are you autonomous or do you need instruction always in order to do something
5) Do you ever take initiative when it comes to doing things
6) Do you do enough to get buy or do you up the ante
7) How welll do you take instruction
8) Are you good role model or preceptor/educator
9) Do you have leadership qualities
10) Too many abscences/call-ins?
11) Do you contribute to improvement or advancement of the team or company?

There are lotsa questions to ask, the above are some of them
