That card appears to have one paper punch and just tie a knot on it

Using the paper punch and threading the ribbon through two holes is a good idea. Here's another cute idea (but not exactly like your card) -- tape a piece of ribbon to each flap of the card, then tie together -- the recipient will have to untie the ribbon in order to open the card.

Tieing the ribbon and sticking on with glue or sticky tape (or a loop of cellophone) also works well.

If you want to make a cute ribbon for sticking on the card, take a length of ribbon. Make a circle with it, then push down the circle so you've got a little bow. You may want to tape the two ends, or at least overlap them. Take another length of ribbon, and tie where the ends meet (you'll have two layers). This will make something like a bow tie. Trim the ends as desired, and you'll have a very cute ribbon tie.

Aw, heck, a picture is worth a 100 of my words -- take a look at this site to see what I am talking about.

Punch two small holes in the side of the card, thread the ribbon through and tie your bow or tie the bow and use double sided sticky tape to fasten it to the card.

It looks like one on my bookmark. Punch one hole. Insert a doubled ribbon thru the hole. Then tuck the 2 loose ends thru the ribbon, and cut it off short. Hope this makes sense to you.
