You can tell your sister you're going to be the designated driver. You heard the cops are cracking down. You can say you made a New Years resolution to cut back on alcohol. You can drink non-alcoholic drinks, soda, or low alcoholic beverages like O'Douls. You really don't have to explain to your sister why you don't want to drink since, I presume, you are free, white, and 21(that's something my grandma used to say, don't ask me what she meant. She is no longer around and I guess that's an old fashioned expression meaning you were legal to do whatever you wanted).Of course that is no longer politically correct, but you get the point.

Say: Thank you, but I'd prefer water tonight.

Can't believe all the answers telling you to make up a lie when nobody should have to explain why they don't want to drink.

Simply say that it has been upsetting your stomach and messing with your moods. Everyone should take a break from drinking sometimes.

Tell them the truth. They will understand better with that than a stupid lie.

If you lie, it will come back to haunt you later and your friends will not think much of it.

Or, just not go to the thing if you feel you have to lie.

Tell her you have a sinus infection and are taking antibiotics. You are not supposed to drink while taking antibiotics.

I guess you will have to "fib" a little. Tell her it's been giving you heartburn lately, or something. Being a woman, tho, She will probably eventualy guess, anyway.

tell her that you have a headache. or, if you are clever, somehow put the alcohol in your mouth, but do not swallow. be the designated driver. tell her that you gave it up to see if it makes you lose weight.
