Clorox Oxi Magic (found in laundry detergent isle) is the best I've found so far it removes all stains from my active little kids clothes. fill your sink with hot water and put a scoop of oxi in and let it soak for a couple of hours wash in washing machine and it should be fixed.

Depends on what the stain is and what the background is. If the background is white, then bleaching may help. If it's not white and the stain is dye from some other part of the shirt or another piece of clothing, then you're out of luck probably. If the stain is organic, then additional washing may help. If it is as you suspect that you mixed something the color of the stain which was not color fast in your wash, you are likely screwed, yes. Read labels when you wash, especially the first time you wash until the dye has faded a bit.

If your shirt is white, try the new bleach pen. If it has color use oxy clean, The oxy clean comes with directions on how to remove a variety of stains. But because you put it through the dryer you will have to let it soak in a bucket for a few hours.
If it looks reddish like rust, there are alot of rust out products for clothes in the grocery store.

It could be from your detergent, or from your fabric softener. Make sure you add the soap, and then let the tub fill up part way before adding your clothes. Don't just pour the soap on top of your clothes. Make sure you are adding the fabric softener to the water on the bottom of the tub and not directly on your clothes during the last rinse cycle.

It's possible that because you mixed colours it got messed up. But sometimes cling free or bounce sheets leave a nasty stain on our clothes. Wash it again, alone, with a good detergent and see what happens.
