it means, telling the truth, that they arent "playing games" as in, "are you real?" all in all it means, being honest, and nothing less than that. at least thats my take on it.

When someone says something is real, they mean that whatever it is or who ever it is , is not phoney or just doing something for show, or that thing is very true. For instance, when a guy says something and another guy says, " Yo, thats real." what he means is what that person is saying is very true and has a very deep meaning to it. Or when a guy says his girl is "real" he means that she is not pretending to be something shes not, shes really acting like herself.Thats the best answer I can give without throwing in more slang.

–adjective 1. true; not merely ostensible, nominal, or apparent: the real reason for an act.
2. existing or occurring as fact; actual rather than imaginary, ideal, or fictitious: a story taken from real life.

when something is 'real', they could mean a couple things. The first is that if someone says 'keep it real' that means in a blunt way, stay who you are and stay down-to-earth. i've heard some people say 'that's real' and that means that something is cool, or interesting.

Now that's a vague question. Many things are real so I am having trouble understanding your question.
