It's not a scam but you do pay to be in it and everyone else you see in it has paid to be in it.

Yes and no. As someone who was asked to be in it, I can say that I do not regret turning them down. Nobody reads those books (even the adult and professional versions of Who's Who) and they seem to be of no help in college applications.

Pretty much, the only people who actually buy the expensive book are the people who pay to say good things about themselves in it.
