take her out to dinner and give her a pretty diamond necklace or a type of perfume she likes, and lots of her favorite flowers,
or take a nice hot bubble bath w/ her and wash her down

Think about what your wife likes and appreciates. What is a nice surprise for one person would be a nightmare for another. You could plan a nice night out at an upscale restaurant; have her dress up, but don't tell her the destination, just take her there; some women like it. If she likes flowers - get her flowers. Certainly get her a nice card. For a gift, you'll have to improvise; you might have some idea of what she is really into - get her something that reflects this personal attention. Or, you can go for a generic gift like jewelry - again, if she likes it. Bottom line is, you really have to know what she likes most; there are the stereotypes (flowers, jewelry etc), but some women are actually not into this generic cr*p; so - personalize. Maybe there's a movie she's wanted to see - you could plan a movie night. Or a concert/show coming up that you think she might enjoy - get tickets and include them with the card. Good luck.

The gift would have to be of your choosing based on what you know she would like and what you can afford. As for the activity a friend of mine once did this and it was very memorable for his wife and very romantic. When she come home from work on her birthday he was gone. There was a balloon on the table with the words "Follow the balloons" on it with magic marker. At the front entry way one said 'two blocks east' there at a fire station one said 'one block north' and so on. The last one was at a restaurant doorway and said "In here, Happy Birthday!" Inside her husband and a number of close friends waited, the table had a beautiful gift in the center and waithresses and the manager and the chef encircled the table applauding. She loved it! You can use as many or as few balloons as you want and if east or north confuse her you can tell her to gio to a certain street corner or shop she is familiar with. Try something like that if you like it.

Do you live in the city? That would help to know.
A fun thing to do would be to take a limo ride to a nice restaurant and to see a show afterward.
If in the big city - a broadway show would be great. It shows her that you are willing to try new things and expand your repetoire as a romantic. Even just a limo ride to a nice restaurant and then go to the top of the Empire State Building.
If not in the city - check out your local listings for what's happening in your community theatres. Sometimes you can see great shows for a fraction of the cost of NYC. Either way, a limo ride and a show would earn you beaucoup romatic points.

Think about things she's said offhand in the past year about things she'd like to do or have & work with that. My husband knows how much I hate the cold, winter, etc & he just started a new job & leaves earlier than I do in the morning so he can't start my car in the mornings anymore. So he got me a remote car starter for x-mas. THAT'S what I'm talking about - listen to her and go with something that shows you are listening & hearing her & it won't matter what you get her. You live in NY? Take her skating at Rockefeller Ctr. and have a romantic dinner at the restaurant at the top after. Activity & dinner = I very romantic evening & one well cared for wife. She'll love it!
