either from the company you got it from or if you don't rememeber try jostens.com they have tons of class rings

Contact whoever made the original; they may even have the order specs still on file. Jostens is the largest (but not the only) maker of scholastic rings; if in doubt, ask the school who the designated supplier is. (Most schools have an exclusive contract with one company.)
