Whether you are talking concrete or screeded sub-floor, the possibility is that it is a conjunction of wear through trafficking, or a poor mix when initally layed. Cement floors, unless they are specifically granite, specially hardened concrete, are not wearing surfaces, they are put down with the intention of having a floor covering on them.

Probably a mixture between temperature and humidity/moisture. And a result of using the wrong cement mixture.

Cement is meant to be a somewhat pourous product. It shouldn't have large holes, but it will always have a slightly pitted surface.
