Yes buddy, but not always. Some times it hurts during the digital rectal examination.

***Position :
-A man is often examined while he stands, bending forward at the waist. A man can also be examined while lying on his left side, with his knees bent toward his chest.
-A woman is often examined while lying on her back on an examination table, with her feet raised and supported by stirrups. A rectovaginal exam is often done for women so that organs in the pelvic area can be evaluated.

***Risks:
Slight bleeding from the rectum may occur after an examination, especially if hemorrhoids or a breaks in the skin around the anus (anal fissures) are present.

On "very very rare" occasions, some may experience a loss of consciousness (called vasovagal syncope) because of fear or pain when your health professional inserts a finger into the rectum. This is more common in standing up posture.

It will probably hurt a little bit, unless you're used to having things inserted into your rectum. If you're afraid of it hurting, do one yourself. Buy a rubber glove, and some k-y jelly, and stick your finger in your anus and feel around. It will hurt only a little more than that, and only because it's someone else's finger. IF you relax enough, it will not hurt at all.

You'll be on your back with your leg in stirrups and knees spread apart somewhat. The doctor only uses one or two fingers which will be lubricated. Assuming you are normal and have no pathology inside, there will be very little...if any discomfort...just pressure mainly. The doctor will be pressing on your belly as he has his finger(s) in your rectum to feel various organs in there.

Tense belly muscles increase the discomfort. It will be easier for you if you make a point of taking deep, slow breaths during the procedure. That will help you relax your belly muscles.

Edit: My comments are based on the assumption that you are having a manual exam. You didn't specify that you were having anything more invasive than that, i.e., colonoscopy, etc.

I'm a nurse. No it isn't too bad. You will be on your knees, this is the usual position.

You will feel pressure, but no pain you will probably be on your knees.
