I work as a Health care assistant, looking after elderly patients in their homes, the problem is that as soon as people hear I'm South African they start talking down to me, and make assumptions like that I never had any decent schooling.
They will say things like: "How would you know, you dont have this or that in Africa!"
I really cant stand it anymore!
What should I do? Say something back to them or just quietly ask for a transfer?

I work in home care as well, though I am a data entry for a home health provider service, and I do not actually go to the patient's homes, but I do know exactly what you're going through. I'm often told by patients that call our offices that I don't know what I'm talking about because I'm not a nurse or a doctor - I have no actual in-field medical experience - only the paperwork, billing and coding training necessary to do my job.
Where you are from doesn't matter and you can point out that you've had the proper training so that you can do your job properly and give them the best care possible, regardless of your background. If they are still unhappy with you, it should be their responsibility to ask that you not return to their homes. But you shouldn't blame yourself or let their words hurt your feelings. You are doing your job and their opinions, while hurtful, are often times their way of dealing with their declining health. I know that I've personally had patients completely shatter my feelings only to have them crying in my ear over the phone moments later over their frustration with their helplessness.
I can only tell you to do what is best for you, but you must stay strong. Health care can be a difficult field - for everyone.

Well, just say politely and with a smile - "We have it there ma'am and I very well know how to use it. Perhaps you have outdated or media-biased ideas about South Africa." Your calm confidence and composure will surely win over their prejudices.

I would just smile and keep doing what you do. It only bothers you cuz you let it. Or if you prefer then say something back but nicely and reasonably, or if you think it's unchangeable then move on.
That's what I would try, in that order.
