there's a form to be filled up in order for married couple to divorce. It's like signing a contract to certify that you agree with the terms that you made with your wife (such as providing enough money for your children's education) and that making legal (accdg. to the law) to marry/have/copulate another woman.

Seek out a family law attorney or a para-legal to fill out the proper forms for a divorce. It's best not to go to court PRO-SE (on your own) if you do the opposing counsel will squash you like a big bug!

It is not question of filling up of a form of a paper but a matter of mental and social liabilities. Question of divorce arose when a marriage is held. If the marriage is a social contact there remains clauses how to execute a marriage and how to divorce it. But in the point of view if you are a muslim you will have to obey the rules and regulations as guided by the Holy Quran and Hadith. As per Quranic law divorce is permissible but it is quite neglected by the creater Allah. As per quranic order if any body failed to adjust him or her with his wife or with her husband he or she may execute the divorce in three steps. In the first step if any one impose 1st phase of Talak(equiv one third of divorce)both of the conjugal have to remain in the same room because if they bodily meet the imposed 1st phase of talak will automatically be cancelled. After one month if it stands as it was before he may impose 2nd phase of talak. After that if they come adjustment physically the 2nd phase of talak will also be automatically be cancelled. But if it is found that after taking time and personal steps for adjustment both failed and the condition is not adjustable at all in that case one can take third step and final step of talak or divorce after one month and after that question of divorce will be fulfilled and be finalised. It requires a declearation and there should be at least 2 witness. In some of the muslim countries and non muslim countries these quranik rule has been taken in the constitution as the religious rule of different religious custs. If it is a matter of Hindu or cristian there is rules as per religion. other than religious rule there remains state rules and in that case question of filling up of form is an official work. But we should consider this matter as a social based matter not like other official work. A man should always try to make conjugal harmony and hearty atmosphere and should try to refrain from divorce like work. let us make the world happy starting it from our personal and conjugal life taking it as duty of humanity.
