# has been use for a long time as notation for the weight measurement of a pound. The abbreviation "lb" when written by hand can easily be mistaken for the number "16". I don't know why that symbol was chosen, but at the very least it is harder to mistake.

# is also used to abbreviate the word "number." There is probably a good reason for this one too.

You must use the context the symbol is used in to decide if it means pound or number.

It's a symbol. If it has a name other than 'number sign', I don't know what it is. On a telephone, it's still a number sign. However, because people are lazy speakers, they use the term 'pound' because it has recently come to be used to refer to weight instead of the more usual abbreviation 'lb', from the Latin word for 'pound'.

Usually it's "number" or "number sign.". When relating to a phone number it's "pound" or "pound sign."
