I have been asked this riddle but he is on an escalator...it is because the elevator stopped because of a power cut and she was on life support.
However, in real life this would never happen because they have special precautions.

The only thing keeping his wife alive is life support, and the elevator stops because the hospital's power has gone out, thus guaranteeing that the life support has failed and his wife is dead.
