The commercial anti-spyware and antivirus programs are designed to get the most common threats. They can't get absolutely everything. In addition, many programs have legitimate uses (even if they're used illigetimately) and therefore are not picked up in the scans. Keylogging programs and other hacking tools are often in this category. So are many remote control programs (such as dameware).
If you've truly been hacked, getting rid of the programs that did so can be very difficult unless you're an expert (and even if you are). There are free "anti-keylogging" programs out there to help, but I suspect you'll need to manually trace and remove (a very, very difficult task), or wipe your drive and reload.

Try anti-adware software and virus protection, and try lots of them, each have their own strengths so one might remove what three others couldn't.
