Can a man of 40 years old have a small amount of pre *** come out of his penis during the day and not even realize it? I mean no thoughts of sex or physical interactions? Or does maybe a small amount of it come out when urinating, or not having sex for a week? Very important I need to know..

yes. it can be caused by the penis being stimulated just by friction against underwear fabric.he could be totally unaware of it, as it could be an involuntary action.

Yes, does happen. Kinda of annoying when it does, can make your urine stream go off-target! Generally though it is more likely caused by thinking of sex a while earlier, and the release happens or may continue to take place for minutes or even over several hours. Just a guy thing. I can answer a question on this forum, not even thinking about it in any sexual way, , and find out later some flowed. Just the nature of the beast.
