Life is short and precious. We're all going to be dead a long, long time. You will regret not experiencing this delicious opportunity in the years to come if you don't do it. Go for the threesome. You can always take out a restraining order against psycho-girl if she becomes a pain in your neck later.

She probably won't end-up stalking you. This isn't about you. It's about the threesome experience for her and her girlfriend. You are just the method of getting to that goal for them.
Should you do it? If you're not into her than why would you? That is just selling yourself out.

Do it because you may never get another chance to and you'll never forgive yourself for missing the chance. She may be a cling on, but who cares - there's no strings attached, so do it and enjoy. I'd suggest a couple of drinks first thought to loosen everyone up. Good luck to you.
