Perhaps at small restaurants and 2 star hotels you might run into this problem, but in most parts of the country the tap water is now fairly safe (just tastes horrible with all the chemicals and chlorine). Besides almost all Mexican homes and decent restaurants use not only bottled water, but "microbicides" to wash fruit and vegetables to ensure that their customers stay healthy.
In a 4 star hotel you have nothing to worry about in Mexico. The staff at resort areas are well trained and no decent hotel wants to make ANYBODY sick. Mexico during the past 20 years has evolved into one of the most educated and service oriented resort areas in the world. You will most likely have less chance of getting sick from hygiene problems down here than you would in similar places in the USA!

You should inquire at the front desk, and also from the Maitre' de (if there is one).
Most quality hotels in Mexico do NOT want their guests to get ill, (or they'll never come back) so ALL water supplied to the hotels (including shower water) is filtered for your safety.
I have stayed in good quality hotels all throughout Mexico and have NEVER had a problem.
Do beware of salads and such in small restaurants. And beware of ice cubes too in small places...

DO NOT eat the fruit or salad..or drink the water. Your body may not have the antibodies built up as the locals do to handle the bacterias found on the foods and in the water. It doesn't matter how many stars a place has (in Europe having 4 stars only means there is a restaurant in the hotel...um that can mean breakfast served in a dining area...breakfast of "assorted meats and cheeses."
The hotels hire the locals, so they can work at any hotel and live in a Hepatitis-infested dump. All it takes is someone with Hep-A not to wash his/her hands after using the restroom and their disease can be tossed into the salad or preparing/displaying the fruit. Also, beware of the meats you eat as well. Don't eat from a buffet. Order something freshly cooked. And order your water in a bottle, that YOU open. Servers are use to these types of requests. I lived in Mexico. I know they reuse bottles without sanitizing them and the bottled beer may actually be from a draft poured into a bottle from the last guy who sat at the table before you...even combined remainders of different customers non-emptied beers. It sounds nasty, because it is!
Remember, they don't have the food growing/processing restriction watchdogs as the US, UK, Canada or other countries have. So, they can fertilize with anything...and do. My friend and her mother almost died from eating peaches. They had so much insecticide on them that it poisoned them. They spent many weeks in a hospital with their family not knowing if they would survive.
Its your vacation and your life. Its worth a little more money to spend your time having fun and not in the hospital or on the toilet!

4 star hotels in Mexico are OK, you dont have to worry at all specially if you are going to a very visited place like Acapulco or Cancun, they love to spoil guest in order to get a tip.
Dont be concern just take care of small restaurants or small inns , and dont eat shellfish or fish in little shacks or restaurants that could be a very big mistake

nobody makes a good bussiness by getting people sick. Alot of it is mind over matter, if you are worried about getting sick the whole time your there, then you probably will! I know people who only ate in "the best" places and got sick. Use your judgement if it look clean then it's more than likely a good place to eat. Don't skip the small places, they usually buy fresh everyday, and have the tastiest food. MOST PLACES USE PURIFIED WATER AND ICE

You will be fine. In fact if you want to worry about that, you can worry about the salad in the US.
All fruits and vegetables in Mexico are organic and not bio-engineered.
