You need to desperately want it more than any of your classmates, then putting in the hard miles will come by itself.
Don't ask yourself "How much do I need to do?" but say "Let's see how much I CAN do."
Isolate yourself and DON'T STUDY WITH MUSIC - it occupies brainspace and causes interference with your brain's study patterns. Remember, your brain is a computer; the more "files" you have open, the slower it is.
For MATH do lots of problems to reprove the math to yourself and create fixed patterns in your brain.
For PHYSICS know your formulas very, very well and where to use them. When given a physics problem write down the symbols of everything given in the problem and the symbol of what is asked. Then see which formula is most useful.
For BIOLOGY draw up diagrams/pictures to see where everything fits in. Whether it's a species within a family, the appendix's position in the digestive system or a biochemical in the Calvin cycle....everything form part of something bigger in the study of life. Also memorise words by association. A bit of latin knowledge helps but that takes time.
For GEOGRAPHY study with a map. The first thing you need to see in your mind when asked a geography question is a MAP. Use various maps and draw your own while studying.
For HISTORY draw timelines as you study. Comparing these timelines you'll se which things happened at the same time in different areas of the world, who lived in the same time and what influenced what. Try to piece together as big a puzzle as you can. Also use maps and don't forget the small detail - historians LOVE small detail.
For ENGLISH and other languages know your grammar rules and be creative in creative writing while applying correct grammar. Also in essay writing, remember to stay in the same tense (past tense is easiest) and learn some lesser used vocabulary to impress the examiner. Spelling is important - you lose points for wrong spelling. Reread your questions before handing in.

In general leave yourself enough time to study through all your work 3 times. Once is good enough to pass but you want an A+.

Secret tip: No matter if you've finished or not, stop all studying at least one and a half hours before your exam. Relax, go for a walk, dont even rerun stuff in your brain. Give the knowledge space to settle down, so when you walk into the exam room, you are completely relaxed and focused.

You don't say what grade you are in. Obviously, study. If you are at the college or university level, check your library to see if they have filed the old exams. Use the common questions as a study guide.

Obviously study like crazy. Make up very hard tests and test yourself.

If it is an essay type test, honestly it doesn't hurt if your professor can put a face to your name. Suggestions on here are trying to be funny, but schedule at least one session with a prof each semester to "make sure you understand the assignment" or something equally trumped up if you don't really need to meet. This advice is assuming that you behave intelligently in class so that the face and name he/she puts together are a positive image.

written exams- study a lot!!! develop study habits! memorize by mnemonics or acronyms or whatever method you use...

oral exams- be witty. be creative.

relax.

study, cheat, be creative (if it is oral exam) and the most important attend classes, be active but not with stupid questions....
theachers will remember your face if you attend classes!!!
