You may be looking for more of an answer than this, but I'll offer what I do know. (Maybe others will offer more.)

I am not getting this information from any particular study or report. It is just something that is fairly well publicized in general.

People of lower incomes can have more health problems because of the stress of trying to live on not enough money, other stresses associated with some lifestyles (kids can get in trouble in bad neighborhoods, which causes severe stress, etc.)..

People may eat macaroni and bread because it is affordable. People who may be stressed out or depressed often don't have the energy to cook healthier meals.

Stress causes elevated cortisol levels, which causes high blood pressure, elevated blood sugar levels, circulatory problems.

People of lower incomes don't have the time or money to engage in many of the sports or other physical acitivites which offer a lot of exercise.

They may not have health insurance and/or they often do not seek treatment until a condition is very serious.

People who are on "welfare" health insurance may at times not get the same type of treatment that someone on private health insurance or someone who can easily pay for what insurance won't cover would get. For example, the person who cannot afford to pay a couple of thousand dollars to save a tooth may be advised to just have it extracted.
