it started as an old nursery rhyme ... no indication when it was first used
(it used to be doctor not dentist so the toothache thing is out, and I think it's based more on the idea of apples being healthy than a reference to cocaine or anything else)

It came from apples being good for your health, and plus they naturally help to clean your teeth....so in a way you were on the right track with the whole teeth thing, BUT you totally lost me iwth the cocaine drops for toothaches, lol, who told you that crap and how would that relate to apples???

I think there's a person who eat an apple a day .. and the person realize he / she never been sick before .. and don have to go to doctor .. so the person invented : an apple a day , keeps the doctor away ..
Then people use this too .

I think mothers started it to get their children to eat fruit and not just candy becuz children need a healthy diet and a lot of kids are afraid of the doc becuz that means shots.
