I'm sorry it is ill. This sounds like an unfortuantely common combination of problems that generally signal that the basic cares need to be adjusted.
There is a lot of misinformation about turtle care, but the truth is that turtles LOVE space, warmth, and the sun, and need cleanliness and a varied diet for health.
Space- aim for at least 10 gallons of swimming space per inch of shell length.
Warmth- warm the water to 75-80F, and the basking sites to about 90F
Sunlight- turtles bask, partly to obtain UV-B rays from the sun. Since these rays do not penetrate most glass or plastic, we need to offer lighting that includes it- usually a reptile bulb that brags about UV-B (UV-A alone is not helpful).
Cleanliness- use a filter system about 2-3 times stronger than you would for the same size fish tank. Even then you'll have to do weekly partial water changes.
Varied diet- offer about 1/2 good pellets, and the rest live or frozen/thawed 'fish foods' like worms, small fish, bugs, shrimp, beef heart, etc.

You need a reptile vet. In the meantime, make sure he is in clean water, and make sure it is the right temp, warm. Many people keep their sliders too cold, or do not keep their environment clean.
I'm suggesting a vet because sliders a reptiles, and reptiles hide illness until they are almost dead- a good reptile vet can examine your turtle, do certain medical tests, and 'tube' feed if necessary.

I read many very very wrong answers here from people who seemingly know NOTHING about reptiles, but speak with such self confidence as if they had dealt with them their whole life.

If his body is motionless, he's most likely dead. Take him to the vet if you're not sure.
