They do not always. In the space station, or in the space shuttle, for long mission, they do have access to a vaccum toilet system.
But during the launch (and launch starts long before the engine start, we are talking about HOURS here) and landing, the astronauts cannot get off their seat, so, if they have to go, they have to go while "staying".

So they can pee in their space suits if they have to. Most manage not to, but you know, some of those space walks can be 6 or 8 hours long, and they don't get bathroom breaks. However, when in the shuttle or space station, they don't wear diapers. They just use the toilet in the shuttle or space station. Or little plastic bags if the toilet is out of order.

Actually, not all the time they have an emergency comfort room in the space ship. It's time for them to wear diapers if they are raedy to go out of the space ship and explore the other planet.

because in space, there's no gravity. and if they peed in a toilet, the pee would fly everywhere. diapers, however, stay on your butt. pee on astronauts.
