When you arrive for your exam, you will need to undress completely and put on a hospital gown, plus usually there's a paper drape that you put across your lap. Usually they let you keep your socks on, the stirrups are cold.
When the doctor comes in, there should also be a nurse or assistant there to make you feel more comfortable and as a chaperone if the doctor is a man. You'll get up on the exam table on your back with your feet up in the stirrups, and the doctor will ask you to slide down as far as you can so your butt is at the end of the table. You'll need to open your knees wide so the doctor can examine you with a speculum, it's kind of a vaginal spreader. This goes inside you and gently spreads you open so the doctor can see. You'll feel cold and some pressure, but you should not feel any pain, if so, tell the doctor. They will take a swab from your cervix to do a pap smear to check for cervical cancer, these are nearly always normal.
Then the speculum will come out and the doctor's gloved hand with KY jelly will go inside to check your vagina and the size of your uterus. His other hand will be on your belly so he can tell what's going on from the inside and out. At this point he will also do your rectal exam with only one finger, again with KY jelly, it's gross but it should not hurt. He's checking for anything that doesn't feel normal, again 9 times out of 10 everything will be fine. Try to relax your muscles, do relaxation breathing. Long steady breaths, in through your nose, out through your mouth.
If you have any questions, make sure to ask, this is the time. Have the doctor explain anything you don't understand. Try not to be embarrassed, that's what they're there for, and it's your body so you have a right to know.
For the breast exam, you will still be on your back, but they will cover you from the waist down and uncover your top. Using his flat hands without gloves, the doctor will press gently on your breasts and check each one all over, also your armpits. It should not hurt. He's checking for lumps and he may ask you if you do this at home. It's a good idea to do it once a month right after your period so you know what feels normal for you, and you have a better chance of catching a lump early.
For the mammogram, you will have to undress from the waist up and the lab technician will give you a gown to put on. Then she will press each breast, one at a time, between 2 pieces of hard plastic and X-ray them, again checking for anything unusual. Each breast will be pressed from the top to the bottom, and from side to side. This part will be uncomfortable, some women find it painful, some just feel pressure. During the actual X-ray you have to hold your breath, but the rest of the time the relaxation breathing will help. They will ask you to wait while they make sure they got a good picture, that's why it's important to hold your breath and don't move, otherwise the X-ray will be blurry.
For the ultrasound, if it's your breasts, you will need to undress from the waist up, if it's your belly, just pull up your top and pull your pants down to your hips. You'll be on your back on a table and they’ll use a special gel on your skin so the instrument can slide around easily. It uses sound waves to bounce around inside you to see what's going on and shows the picture on a screen. It won’t hurt. For a pelvic (belly) ultrasound usually you need to drink a lot of water beforehand so your bladder is full when they do the exam. The doctor's office should give you instructions for that. You will have to pee but you have to hold it. The sound waves need to bounce off of something in order to show the picture correctly. If you’re pregnant, you’ll be able to see your baby and possibly find out the sex.

Ugh, my past gyno's never did that but last week this one did and it was really fast (matter of seconds) and you just feel a little pressure. The breast exams are not bad at all, you just lay down and they feel around to make sure they do not feel any lumps and they may have you sit up and put your arms above your head or something to make sure that the shape looks normal. I've never had a mammogram, I'm 24 and most of the time they don't start doing those until you are in your 40's.
