It depends on what position it's in on the top of the water. If it's upright, it's just breathing. That's what Bettas do. Any other position, sideways or belly up, it's sick. If that's the case, do some research to see if you can save it by giving it medications or creating a different environment, etc.

Usually not if they are healthy and in good living conditions. They will go up to gulp some air but usually will swim around everywhere. If yours is staying near the top and not building a nest, then he may be cold (check the water temp) to make sure it's close to 80F degrees, make sure the water is clean so ammonia and nitrites aren't collecting. Check him over carefully too, often if they are infected or ill, they will stay close to the water surface.

If he seems to be stuck to the top of the tank and can't seem to get to swim down and stay down without floating back to the top he has swim bladder disease. Get some JUNGLE FUNGUS ELIMINATOR. It clears up fungus and bacteria fast. You will notice a difference in his swimming control in a couple of days.

You may need to put an air bubblier in the tank a lot of fish go to the top of the tank because there is not much air in the tank.

is he sideways? does he look ok otherwise? he could be constipated or have swim bladder disorder. this is going to sound weird but try feeding little bits of green peas. take the outside off and chop up the insides til its the size of his food. you can try to feed him with tweezers or forceps -- you don't want to just drop it in and let it sink to the bottom.
