I need Help With Sims Body shop :(?

Hi Ive tried loads and I CANT work out Sims body shop :( I would love to create my own sims but i just Can't understand it :( I tried using photoshop with it as someone said you had to but that confused me more AM I TOTALLY DAFT Is it really that hard? hope you can help xxx
