This question isn't very clear; you could take it several ways.

For a mutation in an individual to be passed into the gene pool, it must be a mutation in a reproductive cell. A mutation in a body cell (for instance, a mutation that causes a skin cancer on the arm) will not be passed on, so it won't get into the gene pool.

For a mutation to come into a gene pool (as in gene flow) one or more individuals with the mutation must move into the area and reproduce.

essentially a mutation can occur from numerous factors not all of them related. Everything from environmental changes, pollution, radiation, to television can cause mutation in an biological entity. the circumstances behind each mutation are different for each speices.

Evolution in and of itself is a form mutation in that a species evolves slowly to adapt to its environment.
