I had a 2nd degree tear with my first baby.
I birthed over an intact perineum with both #2 and #3.
Good prenatal nutrition is important. It will help you have more elastic skin.

You can do it yourself or have your hubby help you.
DO NOT push flat on your back. Being on your back is the WORST position to push in. It makes you more likely to tear.
Squatting is a GREAT position as it widens the pelvic outlet.
If baby is coming fast, you might try the side lying position.
Talk to your caregiver about avoiding a tear. Ask him/her to use warm compresses on your bottom, do perineal massage on you and support your perineum while you are pushing.
Consider waterbirth. The warm water helps you relax and stretch.

It is really hard to say for sure, because everyone is different. You can hear all kinds of stories of what happened to others but that does not mean you will have the same outcome. For instance we don't know why you tore in the first place. Was it because the head was to large and your skin did not stretch enough? Or, did you possibly have an epidural (or just out of inexperience) and not push correctly, and actually fight the babies decent. In my case I was the latter with my first. I had an epidural, could not feel anything and being inexperienced and not knowing how to push correctly, I ended up actually fighting it and not know it. I had 3rd degree tears and tons of stitches. I had trouble sitting and trouble with my bladder for weeks after his birth. He was 7lbs. My last 2 were nearly 10 lbs I had no epidural and was able to focus on pushing properly and did not tear at all. I was in church, sitting on the pews less than 24 hrs after my daughters birth and other than some mild discomfort was generally pain free after their births. Just mention this to the nurse and you OBGYN at the delivery, they will be able to monitor you and if it looks like you will tear they will do an episiotomy, which will heal much nicer than a tear any day of the week! You can tell your doctor that you tore the last time, and ask him if he can cut you this time(episiotomy)(it doesn't hurt they numb you and you don't feel it) I had an episiotomy and i wasn't in a lot of pain but I've heard if you tear it's very painful(you should know) and hard to stitch back up. Good luck with your delivery!

my first labour ended up with me have 11 stitches. i gave birth laying on my back. my second baby i was on all fours (like doggy style) and i didnt tear at all and he was nearly two pounds heavier then my first baby. i think the position your in has a lot to do with it. let gravity do some of the work.

I had to have 3 stitches with my first, and no stitches with my second (there was a small tear up high that could not be stitched but it wasn't bad at all)

My sister had an episiotomy with her first and a midwife who massaged and "stretched" her so she didn't need one with the second and she said she would rather be cut any day. Per her, she couldn't stand up straight for two weeks.
I was cut, and it wasn't so bad. I think you will be pretty sore down there regardless,

try massage before you go into labor. I didn't know about it until I was actually in labor, my nurse did this for me and I only had a teeny tiny tear http://www.suite101.com/article.cfm/pregnancy_childbirth/43415/1

Do kegel excersizes. No it wont be more stretchy. You need to do the kegels. Its where you pretend like your trying to stop peeing. do it over &over several times a day.
