Nothing beats just practicing. Keep Playing the A chord and then changing to the D chord over and over again. People say there is something called 'muscle memory"....regardless of whether that really exists or not I can certainly say that if you just keep pratcising suddenly things will just work out without you even having to think of where your fingers should be moving too. It has surprised me often when playing the guitar...something that seemed impossible when you first start out suddenly becomes something you can do without thinking at all. Practice practice practise. If there is no such thing as 'muscle memory" there sure is something that suddenly "clicks" and you do it without thinking!

Practice Practice Practice.
When you are in A notice that your 4rd finger its on the 2nd string 2nd fret, when switching just slide your 3rd finger from fret 2 to fret 3 and just bring your other fingers down.
Trying doin these with other chords.

It just takes practice and repetition. Bar chords is the other way to switch. It just takes time to get your mind used to putting your fingers in a certain way. Keep it up and crank it up!

Hope this helps
