In the series, the process pretty much mirrored the process for military academies today... you have to meet physical and academic standards (and of course the higher you score on your exams the more competitive you are). You also have to be sponsored by someone in Starfleet (mirroring the congressional nomination you need to get into today's academies).
The way it was depicted in TNG with Wesley (ick)... Federation planets conduct testing for applicants, and only the highest scoring applicant goes on to the academy. I always assumed that certain planets have more than one slot that can be filled, and the planet Wesley tested on for whatever reason just had one slot. Otherwise, it would be insanely hard to get into SFA if you are on Earth and you are competing against everyone else on Earth that applies and there is only one spot up for grabs. That would mean every entering class only has one freshman from every planet... doesn't sound realistic.

I assume you are talking about in the Star Trek show and not in real life. If you are, then you have to apply just like any school in real life and then you have to take tests and compete against other applicants to gain entry.
