There are many ways a condom can fail, if it is not the right size for the penis using it, if the man goes limp whilst having sex the condom can slip off inside the woman, this also can happen if after he has ***, and stays inside for too long. when he pulls out if it is not fitted well it can squish up the sides, when he takes it off if some drops out and falls on you. I have had a condom come off inside of me 3 times in my life, and a condom has only broke once in my life. My boyfriend actually told me he can tell if a condom is going to break or not, he thinks if they break it is because the man is not being responsible and keeps going, he said he can feel the tension before it breaks, so be careful and use two methods of contraception that way you will be double safe. Condoms can slip off completely during intercourse and not be felt. It happened to us.
Generally, condoms do not leak or spill if used properly (don't stay together after the guy finishes because as the penis goes flaccid there can be leaks around it.

Yes, after orgasm and ejaculation, the man's erection begins to soften. Even just a little softening could allow some of the sperm to leak out the top. So always have him pull out pretty soon after orgasm. Also, condoms break occassionally, so use an extra spermicide during your most fertile times (14 days after your period begins, and 5 days before and 5 days after that). Always use a condom with spermicidal lubricant.

That's why you must leave a space at the top of the condom for the semen to be "caught" at the tip of the condom. Only when he takes it off it should come out.

A condom has millions of microscopic pores in it. Sometimes, a sperm gets lucky and makes it through. That's why it is important to buy spermicidal condoms. That helps zap the sperm. And, this is the scary thing, an HIV virus is smaller than a sperm. If a sperm can get through, I often wonder what keeps HIV out.

I'd like to know that too. That's what happened when I got pg with the one I lost over the summer. It didn't break but it didn't fit right either. Finally after winding up pg again, we decided DH would have a vascetomy. Nothing else works!

a hole as small as a needle puncture can cause a leak that could lead to pregnancy... you don't need alot of sperm... just strong swimmers
