Apply for Several Programs here in the United States. There are lots of colleges looking for Intelligent, Determined students, from all of the world. You need to first be accepted in to a program and then apply for a student Visa and such. Coming from Iran, because of our governments paranoia towards College Aged Muslims Males from the Middle East it will be hard for you to get a visa. But if you want some of the best medical training in the world, nothing should deter from you coming here to study. I also warn you, be prepared for lots of background checks, government surveillance, and a list of things you can and cant do.
One a side note, My government and lots of people in the United States are paranoid of men like yourself because you fit some pre-determined stereotype. In my mind this is wrong, a lot of what has happened in the last few years has been the fault of the United States Government. So I hope you can come to the US study and become a great doctor. Good Luck in your endeavors.

Do not rule out Europe as a place to study. I have heard many good stories of research surgery in Germany. Yes, the U.S. does have some good medical teaching hospitals, but Europe is closer to where you are.
Also, you have to understand the political climate at this time, because for you, it will be prohibitive and certainly not encouraging. Student Visas are very hard to get right now and this goes for all students outside the U.S. who wish to study here. Use your time wisely and continue to learn where you are. Perhaps in time, it will be better, but right now? Who is to say?
