I had to find the phone # of the city hall of the city I was born in, that worked for me.

First you get hold of the county where he was born, either hall of records or county clerk, find out how much a new birth certificate is and send them the check. When you get that, go to the social security office and apply for a duplicate social security card. They each should be less than $20.

If residing in the city he was born in, go to the birth/death certificates office of city hall. If born out of the state or counrty (as I was) you can order a duplicate birth certificate on-line. As far as a replacement SS card, go to any social security office and the representative will verify your identity by asking personal questions such as previous employers, dates, & mothers maiden name, for example. He will then give you a printout that will verify a duplicate card has been ordered which usually takes 7-10 business days to receive. In the meantime, the printout SS paper and the duplicate birth certificate are enough to secure a state ID or drivers license.

Go to the Social Security Office and apply for another Social Security card. Go the Bureau of Vital Statistics and apply for another Birth Certificate. Plan on showing ID for both. I would call both to see what forms of ID you need besides a driver's license or State ID. They are government offices.

birth certificate would come from the registers office in the town in which it was registered. Social security card would be the social security iffice and he will probably need a birth certificate to do this (depending on what kind of mood the lady at the window is in and how many hoops she wants you to jump through and how many times ahe wants you to come back)

if he has pictur id he can go the th ss office and request a new card. when he does that he will get a print out with his name and thge number on it with the printout and the picture id he can go to the county records clerk ad get a new birth certificate.
the only issue will be if he lives far from where he was born
