Your old doctors office can get a copy for you. Maybe they will fax it to your new doctor.

Call your previous doctor and ask for a new copy of the immunization record. You may have to give them a note in writing, but they're legally required to ask for that. Don't expect your new doctor to call the old doctor for you - most doctors won't release them to anyone without the parent contacting them first.
Worst case senario - keep the existing appointment, but call first and explain to the new office that you're waiting for a new copy of the shot record, and would they prefer you to reschedule or have the exam and come a different day for vaccines. My office will have you come for the exam and then do the vaccines a different time if you need to. (Reason being - a new patient has extra time booked so the doctor can do a complete and through history and exam and just vaccines takes A LOT less time than that!)

Just stop by the old Dr and get a copy of your baby's records. You will want your new Dr to have all of his info anyway. If you're embarrassed to tell the people at the old office you're moving to a new one then just say you want it for your own records. I've done the same thing before. Or you could have the old Dr fax it to the new Dr. The insurance card...that can probably wait...it's not so urgent. Whenever that has happened to me they say bring it in next time. Or you could call the insurance company and get all of the necessary info. to take with you. My newborn is 2.5 months old and I haven't gotten a card for him yet. As of this week he has had 5 Dr appointments and they have not given me any trouble about it yet.

Your first safe move if you cannot find the record is to get in touch with the doctor's offices who previously gave your son his shots. If you can't remember or are unable to get these don't fret! Just let his new doctor know that the record has been been misplaced and you're not able to get the info. Your son's physician will know what shots to give despite this, and there is no danger of your son having duplicate shots in most cases. Your doctor will explain this further. I'm a family nurse practitioner- have seen this frequently as people move around so much! Best of luck to you and your son!

Anytime you switch doctors, this goes for any family memeber, you should have your old records sent to the new doctor before the appointment. You'd have to call the old doctor and sign a medical record release form for the old doc to send the records. This way the new doctor has everything he/she needs, all the history and everything.

I work for a pediatrician, we see this alot. I would call right now to his old dr. and have them fax it. In Michigan we have a on-line immunization web site that the dr. office can get his shot record there. I dont know if its in every state. Do you know if hes up to date? If he is just tell them that to buy a couple more days if needed. Good luck,

Go into or call the old doctor's office and have his records transferred. You may have to sign a release form before the records can be sent.

Relax, wherever you got the baby's last shot that Hospital or clinic should have the record. Just tell the Doctor or call that clinic/hospital yourself and ask them to fax your doctor one or send you one or you can pick it up. They'll even send you your insurance info if you ask.........

Don't worry just ask the old Doctor to send it to the new Doc. A funny little FYI I lost mine from when I was a baby my pediatrician passed long ago& I never got mine ;your high school keeps them on microfilm! 15 years later lol!!! Thank goodness! Good luck
