My medicaid for pregnant women is pending and they said i just need to show proof of my childrens identifications who are under 16 by getting a form from the office..i forgot what form it was..im gonna take a guess and she said it was something like a testation or astation something that sounded like that. I forgot..do u know what this form is? Its to sign from me and hubby for our kids.thanks for any help

First I would get all my kids Id make sure it's ready for when you need it . Call the person that told you about the form ask her what form you need. Where to get this form and for a number as well . please don't wait. write it down when you call her so you don't forget it. i hope this helps

if you do not know the correct form call the medicaid office and ask them to re-tell you the correct form and then write it down. sometimes the place you need the form(s) from will send for them for you if you ask ...just don't dawdle on getting the file(s) requested time may run out and you will get put back on the waiting list again for the funds to re-materialize

Call and ask them. They will understand that you were given alot of information that day and that it is very hard to remember exactly what they said abou teverything. Just call and check that way after you get the forms it will be done with and you will have your medicaid.

You could call them and ask if you could come by to pick it up or if you have access to the Internet if it is available on line. Some on line forms can even be typed on line and printed to prevent errors and misunderstanding of numbers and letters. Hope this helped!

I dont' know what state your in so I can't tell you for sure, but if you do a search on Yahoo using Medicade and your state as a search string you should be able to find their site and download the form... and/or call someone to find out what they needed.

in KY they only need your children's state birth certificate, social security card and if your unmarried proof that you're actively seeking child support from the non-custodial parent. so i don't know what form your talking about.
