In safe mode only the minimum drivers are loaded and if a virus is active it will detect it in run mode but may not be able to remove it.
You should run the scans in safe mode once and a while.

In general, Windows does not allow the deletion of files when they are in use. However, Safe Mode will only load a minimum amount of drivers and services to run Windows, making removal a better bet, basically it helps to remove the "tough ones"

they don't always but the reason it is recommended is due to the fact of the windows operating system refusing to allow the removal of a running file/task/program so scanning in safe mode can make detection and removal of viruses/Trojans/spy-ware easier than it would otherwise would be.

I personally would recommend the use of a firewall program to keep it out of your computer in the first place but sometimes that will only slow it down not prevent it. good luck and may your security software keep you out of trouble for as long as you own a computer.
