Which would give me a better chance of getting accepted? Also, if I were to transfer to a 4-year program, would my SAT scores/high school grades be considered? Thanks!

6 year program from high school. You have a defined path into the pharm school.

If you try and transfer, you have to qualify and get accepted all over again. Considering how popular pharmacy is, you have almost no chance of transferring in after just 2 years spent fulfilling the prerequisites. You'd be competing against people who hold bachelor degrees. They already have the young kids who took their 6 year program so they can just put you off until you apply with a bachelors.

Try for the 6 year program. If you don't get in, you've got another shot to get in.

If you have high enough high school grades/ACT, you'll be better off doing a straight 6 year program. It's a little easier to get into.
If you decide to transfer, your high school grades/ACT are not counted.
