put your arm and hand up, there must be a teacher or prof inspecting the exam room. When he/she comes, quietly signal you need some tissue, otherwise, like the other guy said, you'll have to sacrifice the cleanliness of your sleeves. You probably will not be allowed to leave the room to go to a washroom to get paper tissue, besides, that wastes precious time for the exam.

That's what sleeves are for! If you are able to get up and leave, just quietly go to the restroom and take care of yourself.
