if the quotation from a poem is more than 4 lines long and indented, do you still need a partial sentence to intoduce it? what punctuation do you use?

State something like

In John Doe's poem, "My Puppy" he writes:
abcdefgajflajflkjflksf/
faffaf/asfasf
aff/ asf afsaf
aff/afsdff. (456)
use slashes to show line breaks, end the quote with a period, then give the line numbers. Be sure it is double indented, each line for a block quote. Check www.dianahacker.com under student sites and the writer's reference for more help.
