Sometimes they will do a very early ultrasound at around 8 weeks to check for viability of the fetus. This is where they can see two sacks. They usually can't pick up a heartbeat until around 12 weeks with the doppler, though.

Depends on the type of ultrasound. I went through fertility treatments so my doctor used vaginal ultrasounds and they knew at two weeks that I was pregnant. I had them every week, but they didn't have the audio, but I had a scare at barely six weeks and they hooked me up to one with audio and we could hear the heartbeat very clearly. If there had been two babies I have no doubt they would have seen and heard them that early.

Should be able to tell pretty early on. I have twins. I'm not sure at how many weeks/months but it should be easy to spot early.
