It mean's "Hey!," "Hello," or it's used to add exclaimation at the end of a sentence. Like this, yo.

It came from Yo! MTV Raps. Not really. Who knows?

its slang and originated through basically bad manners, basically people went from saying "hey you" to just "yo"
