There are secondary products or impurities produced in many reactions. Also, there may be reactants left over.The yield is really 100% if you total everything up. It is less than 100% if you are looking at just one target product.

because nothing is accurate but the hypothesis a certain value and give what is expected but of course the result wont be 100% the same as what is expected due to conditional, enviromental or any other unexpected variables and these unexpected changes affect the yield

This is because no experiment is perfect, unless you are in a hi-tech lab. There are elements that change during the lab. These elements are not calculated in the theoretical yield.
