This has actually been the joke of a few episodes. I know Brian, the dog, is heard by all and can hear Stewie. I think the other instances of people hearing Stewie are meant to just be them intuitive into what he might be saying, but they don't really understand him. There are times when they repond to exactly what he says and they deviate from that, but I think the intuition things is what is meant and if they have to deviate from that for plot support than they do and they just leave us wondering. Also, he says, "laugh and cry." That is another thing that has been up for debate.

As they say in the end of Petoria--"I don't understand, can the people understand the baby, or what?"
It's left up to you. Brian does understand him, but as for the rest...they don't really understand him. It's the same with the whole dogs talking in 101 Dalmatians--they're speaking two different languages, the dogs and the humans. We can just understand both.
Also, Stewie says "laugh and cry." If you turn on the subtitles, it says "laugh and cry."

Brian can hear Stewie and so can a few other people but Brian is the only one in his family who can understand him.
He says laugh and cry in the intro

i think its both because some episodes they can understand stewie and some they can't...like the other night peter was trying to potty train stewie and stewie asked if they should lite up a doobie and watch some porn and peter was like Yeah?...so i guess it depends on the theme for the night...and its is laugh and cry.

Only Brian can understand Stewie all the time apparently.
And I vote for " 'effin cry" , but I believe it depends which season the episode is from, cause I swear I've heard both.

Seth MacFarlane has asserted that he and his staff will never take a definite stand on whether Stewie can converse with adults. Whether the other characters understand him or not is left to the writers, who can decide which possibility is most appropriate for a given situation

I believe it like this: Stewie talks and you understand him, but his parents and everyone else hear "baby talk and some words". Think outside the box...maybe different frequencies? lol

I think brian is the only one who can fully understand him. I think louis hears it like baby talk and the others just ignore him.

pretty sure that everybody can hear him, but only brian really hears his diabolical mind and plots of killing his mother
