The short answer is "yes".
It depends on the school. It also depends on what you want to do: some theater programs will accept you into a directing track or a technical track (costuming, sets, etc), and don't require auditions for those.
But a good theater department usually has an audition requirement for actors. They've usually got more applicants than slots, and use the auditions to narrow it down.
Often, even directors and tech are required to do at least some acting, so schools often have auditions even for those tracks. As a theater major you'll do literally dozens of shows, and they need many, many actors to fill all of the roles. So they often take people who don't expect to act (and conversely, often take actors and put them in technical positions, because you need those, too.)

At the school where I used to teach, there was no audition required. We offered both BA and BFA degree programs, but all students came in as "general" theatre majors. At the end of their freshman year, they could either: a) choose a BFA path, in which case they'd either have to audition or interview for admission; or b) remain in the BA program.
Not everyone who majors in theatre has a primary interest in performance, so I shouldn't think that any kind of audition would be required.

It very much depends on the requirements of the college or university where you're wanting to study. If you're attending a university, you probably won't have to audition - you'll simply need to meet the course requirements. (Once in the program, though, you will need to audition for roles, which are often a part of the graduation requirements.)
However, if you're wanting to attend a conservatory or acting school, an audition will be required. These auditions are scheduled for incoming students every year - so you'd simply need to contact the head of the program you're trying to get into to get on their schedule.

This depends entirely on where you are. For some colleges and universities that have theatre departments (as opposed to conservatory style training academies), all you need to do is declare theatre as your major and - bam! - you're a theatre major.
I would ask "where are you thinking of going to school?"
