No. Your brain won't get confused. It'll only consume more energy than it would normally. Your heart will beat faster than normal. And your body might try to lift you higher or try to keep you to the ground (which depends on your thoughts). Most of it is caused by the environment around you.
In this case: Your brain recieves most of the information it needs from your eyes. So, if you close your eyes everything will go back to normal (we assume that your way has no blocks in it and never ends).
If another train goes by and you see that other person who's doing the same (we assume you didn't know this would happen), your brain will try to make you go faster or slower (it won't hold the same speed), for a little while. After that you might stop, or go on. These all depend on your mind (not the brain).

I don't know about brain confusion, but I would get sick.
I guess it's like sitting backwards on a lightrail.... Your brain really doesn't notice it much. And if you're focused on your running, you wouldn't really notice your surroundings.
